Required Agreements for Power of Attorneys

The CSOS POA applicant is required to have read and agreed to the following documents before accessing the CSOS application forms:

• CSOS Subscriber Agreement
• CSOS Privacy Policy

I have read and agree to the CSOS Registrant Agreement, the CSOS Subscriber Agreement, and the CSOS Privacy Policy and am aware of the CSOS application requirements as described in the CSOS Subscriber Manual.
